does knuckle cracking cause arthritis. So when you're cracking your knuckles, what's happening is that gas bubbles are forming in the joint that rupture and cause the kind of snapping sound that you get all of our joints are covered by a joint capsule and inside the joint there's lubricating fluid or synovial fluid. And what happens when you hyper extend your joint or distract your joint, You create negative pressure inside the joint and the gasses that are dissolved in that synovial fluid actually become small air bubbles and as they form or as they rupture they create that loud popping sound. Some people get a sense of tightness and stiffness and that sensation for some patients is relieved when they stretch their joints and the popping sensation is simply a consequence of that stretching or traction. There's quite a bit of evidence now that seems to indicate that while knuckle cracking may have some negative consequences. It certainly doesn't cause arthritis, but very aggressive and very habitual knuckle cracking can cause ligament injuries. So it may not cause arthritis but can cause sort of chronic sprains. And there are some studies that seem to indicate that habitual knuckle crackers tend to have somewhat more swollen joints and slightly weaker grip. I've had patients who come to me because they do have hand pain as a result of knuckle cracking. Hand pain can manifest for so many different reasons arthritis being only one of them and a very common one of course, but patients get hand pain due to repetitious use of their hands. Hand injuries like sprains and strains they get tendonitis is uh they get conditions which are very commonly called trigger fingers. They can get nerve compression syndromes like carpal tunnel syndrome, which are also very painful. Osteoarthritis is what most people think about when they think about arthritis and that's age related degenerative joint disease and it's inevitable for most people as long as you live a normal lifespan. However, there's a whole nother category of arthritis, which is inflammatory arthritis is, and these are primarily autoimmune diseases such as rheumatoid arthritis or psoriatic arthritis. And these are treated quite effectively uh in this day and age with medications trying to maintain joint health is all part of trying to stay fit really. And it involves trying to maintain flexibility and good strength. The weight bearing joints of our lower extremities, like our hips and our knees and our ankles depend on good strength for balance for coordination. And frankly keeping down our weight is probably going to be the single most important thing that we can do to try to preserve our lower extremity weight bearing joints. So unfortunately arthritis is inevitable for everyone as long as you live long enough. Although we don't have a cure for osteoarthritis, it doesn't mean that you can't get some relief from those symptoms. So it's certainly worthwhile visiting your doctor um and getting some advice about how to minimize the symptoms and to uh maximize your function in spite of it

can joint pain predict the weather. Well, that's actually a fact, as hard as it is to believe when your grandmother tells you that her arthritis flares up in advance of a rainstorm or a change in weather, it actually can happen. It seems that people who have arthritis have surgical scars or old injuries can actually tell when weather changes about to take place. And the reason for that is because of barrow receptors. A receptor is actually a organ that is connected to nerves that send information to the brain to give the brain perception about the position of joints, the amount of stretch of muscles or in the case of ligaments, whether they're being injured and causing pain. So barrow receptors are mechanical receptors. They're actually end organs for nerves that can actually perceive stretch and we have them in many organs in our body, including our blood vessels, our skin, our muscles and in our ligaments. And what they do is they tell the brain when there are changes in pressure barrel receptors are actually most talked about in the blood vessels of our body. In fact, the great blood vessels like the aorta and what they do is they actually help detect blood pressure and they regulate blood pressure in a home a static way. But the barrier receptors that we have in our joints can help us detect small changes in atmospheric pressure when we have inflammation of the joints or injuries of the joints. These receptors are already sensitized and so they become even more sensitized when there are changes in barometric pressure, such as when a weather front is coming through and rain is imminent. And so these can sometimes elicit pain and soreness. The kinds of symptoms that your grandmother might tell you about when she's predicting the weather change. What's happening is actually the barrow receptors are responding to low pressure because it's low pressure systems that precede a rainstorm or a sudden change in the weather. And what happens with the lowering of the barometric pressure is that the pressure inside the joint actually causes the joint to expand and that stretch is what's perceived by the barrel receptor. And that's the message that's being sent to the brain. And because in arthritic joints or injured joints, the barrel receptors are already sensitized this change in pressure and the stretch will actually accentuate that response and be perceived as pain. It's quite a common phenomenon that patients can actually sense changes in the weather. And it's because of these barrel receptors, but there's no injury that occurs because of the changes in barometric pressure. It's just something that uh that people will feel. But it's also interesting to note that there is some contra and some disagreement about whether these receptors and joints actually do what we're talking about. But I will say it's so widespread and having experienced it myself, I feel like there must be some truth to what we're talking about here. If there are people out there who are experiencing exacerbations of their arthritis because of weather changes. There are some things that you can do to help mitigate those symptoms, and they include just taking over the counter uh analgesics or anti inflammatory medicine or using topical medications. The symptoms are usually mild, and of course, they'll pass as the weather passes.

Does typing on a computer cause carpal tunnel syndrome?

So you've probably heard that using the computer keyboard will lead to carpal tunnel syndrome is that fact or fiction? Well, it's fiction. Carpal tunnel syndrome is probably one of the most misunderstood conditions that I come across. Patients often come to me with pain in their arms soreness in their wrists and they oftentimes are suspicious that they might be suffering from carpal tunnel syndrome. But carpal tunnel syndrome is a very specific problem. It's actually a compressive neuropathy, which means that a nerve is being pinched and it's being pinched at the wrist level. In fact, in our wrist we have a tunnel, the carpal tunnel, which is actually the space through which the tendons that bend our fingers travel, but also the median nerve, which gives us feeling in our fingers and also gives us electricity to the small muscles at the base of our thumbs. And what happens in carpal tunnel syndrome is that space becomes too crowded too tight and the nerve, which is the softest structure in that tunnel becomes compressed and that leads to symptoms of numbness, tingling sensations, pins and needles, sensations, and then late in the disease, the muscles that the nerve innovates will start to atrophy and become weaker except in very rare circumstances. Computer use and keyboard use are not likely to cause carpal tunnel syndrome. In fact, we don't really know why people get carpal tunnel syndrome, but we do know that there are certain predisposing factors that make you more susceptible. For instance, diabetes, obesity, just being a woman as opposed to a man makes you more likely thyroid diseases. And also pregnancy. The third trimester of pregnancy is a time when many women develop symptoms of carpal tunnel syndrome. The vast majority of patients who come to see me with pain in their hands, their wrists or their forearms more often than not, what they're experiencing our overuse symptoms. And those can be things like tendinitis is, or chronic sprains over using any part of the body will lead to pain as opposed to carpal tunnel syndrome, repetitive strain injury can manifest themselves in a number of different ways. It's a condition that's very non specific. There's nothing torn, there's nothing ruptured, there's nothing fractured. But what's happening is there's a mechanical overload of the structures of the hand and arm and that results in pain. It's like any kind of machine part that has a certain tolerance. And if it exceeds that tolerance, the parts start to malfunction and our bodies do the same thing, which is that the pain receptors start to become stimulated and we start experiencing the kind of discomforts that we're talking about taking soreness, stiffness, cramping. So some people who have carpal tunnel syndrome, their symptoms will abate if they use a wrist splint. Sometimes we give cortisone injections into the carpal canal. We put people on nonsteroidal anti inflammatory medication and in many cases their symptoms will abate. Unfortunately, in some circumstances, the condition does not resolve and we worry about nerve damage because of the pressure on the nerve. So what we do is carpal tunnel release surgery and what we're doing at the time of the surgery is we're actually cutting the transverse carpal ligament and thereby opening the carpal canal and decompressing the median nerve. One thing that's been found to be really helpful for those people who work on a computer keyboard for protracted periods of time is actually take frequent breaks. I usually tell patients to take a break at least every 20 minutes, also trying to reduce any unnecessary work that they're doing with their hands. So, for instance, if you're accustomed to going home and playing video games or if you're doing a lot of social texting and emailing for the time being, you want to reduce that as much as possible. We also try to increase their for doing those tasks and a great way to do that is through physical therapy or occupational therapy. A therapist working with a patient who is suffering from hand, wrist or arm pain due to repetitive strain injury. What the therapist will likely do is what's called work hardening. It's trying to train patients to be more tolerant to the kind of work that they're doing and essentially toughening up their hands to be able to put up with those tasks
