[MUSIC PLAYING] PETER SANDS: If you talk about the broader business, community all of companies across multiple sectors, banks, consumer goods companies, airlines, and things, they're not really engaged in global health in the way that they engage in issues like the environment. We looked at the Fortune 500 companies and looked at how many of them had strategies or targets around the environment and how many of them had it around global health, and it was really quite striking-- around the environment, 74% and around health, 10%. And if you stripped out the pharma companies and so on, it was ridiculously low. CAROLINE BUCKEE: From in the context of pandemic preparedness, this is something that companies have to think about. If the Philippines has a massive epidemic, then all of international shipping goes down. PETER SANDS: They don't nearly enough. CAROLINE BUCKEE: Yeah, they don't. PETER SANDS: Companies don't do it nearly systematically enough. The activist community in the environmental climate change space took a very different strategy from the global health community. The activist community in environment space decided to engage with corporates, both collaboratively and also in a challenging way. And they use things like regulations, regulations on disclosures about carbon footprints and things like this as tools to get companies to think more about these things. I actually think that the global health community needs to take a long, hard look at the way it's engaging with the private sector and say, what can we learn from the way the environmental community did it to get that kind of specificity and directness in the interaction? Because at the moment, at the moment, there's deep antipathy and huge amounts of mistrust. It's one of the things that surprised me is a relative newcomer is just how distrustful of the private sector much of the global health community is. And you're just not going to get-- that's a really poor starting point from for any kind of collaboration. [MUSIC PLAYING]

Multinational Corporations Must Prioritize Global Health

A global pandemic could disrupt every economic sector, so why don't corporations seem to care?

What are you talking about? The broader business community, All of companies across multiple sectors banks, consumer goods companies, airlines, things. They're not really engaged in global health in the way that they engaged in issues like the environment. We looked at the Fortune 500 companies and looked at how many of them had strategies will target around the environment and how many of them had it around global health. And it was really quite striking around environment 74% and ran health 10. And if you strip tag the farmer companies and so on, it was ridiculously low from in a context of pandemic preparedness. You know, this is something that companies have to I think about, You know, if if the Philippines has a massive epidemic, they don't nearly enough to really think about, companies don't do it nearly systematically enough. The activist community and the environmental climate change Spain. It took a very different strategy from the global health community. The activist community and vomit told place, decided to engage with corporate but collaboratively and also in a challenging way on they use things like regulations, regulations on disclosures about carbon footprints and things like that as tools to get to get companies to think more about these things. I actually think that the global health community needs to take a long, hard look at the way it's engaging with the private sector and say, What can we learn from the way the environmental community did it to get that kind of specificity, um, and directness in the interaction? Because at the moment, a moment, this deep antipathy on huge meant to mistrust, it's one of the things that surprised me is a relative newcomer is just have distrustful of the private sector. Much of the global health community is, and you're just not going to get. That's a really poor starting point from only kind of collaboration.

A Vision of a High-Tech Smart City

The first phase is expected to launch in 2022. (April 2018)

The Philippine government's been moving fast to combat Dengue. In December, it approved the sale of the world's first Dengue vaccine, Dengvaxia. Now they're going around schools and to target vaccinate more than a million school children against the Dengue virus. Colitas family insists that child grew up to be strong and healthy with no history of any illness. They believe Dengvaxia could be the culprit. Do you have records of how many have allegedly fallen ill cuz I'm reading different numbers? In the light of this new analysis, the DOH will place the dengue vaccination program on hold. [ MUSIC ] [ NOISE ] We at Sanofi ensure each and every one of you that Dengvaxia is and continues to be a safe and efficacious vaccine. Dengue is the fastest growing, most common mosquito- borne illness. It's primarily a problem in the Southern Hemis You're, especially in Asia and Latin America, and each year it infects 390 million people. There are four different strains of the virus and you can get sick from any of them. But usually the way dankey works the first time you get an infection, you don't get very sick. You may not even know that you're sick. With the second infection if it's from a different strain than your first infection, there's a higher chance that you'll get severely ill the cases where people die of bengay most often results from the second infection. So the vaccine against Bengay had to be able to protect people against all four strings. Sanofi thought that it had that with thing maxia Sanofi is a French drug company. It's one of the largest pharmaceutical companies in the world with $ 42 billion in annual revenue. They got interested in dankey decades ago because it's such a widespread illness and because there was no drug to treat it, no cure for dankey and now No existing vaccine for it. Everyone was really excited about this vaccine. [ UNKNOWN ] saw a big public health opportunity also a big commercial opportunity. And there were competitors working on [ UNKNOWN ] vaccines at the same time. So they really wanted to give themselves an edge over the competition. They compressed the Time that it would usually take to do clinical trials. They started their big phase three trial before the phase two trial had ended. They set up their operations commercially to really move as quickly as they could. They invested in manufacturing early, so they would have a ready supply of this vaccine as soon as it was approved. Meanwhile, health ministers were excited about the possibility of this vaccine too. The Philippines was a really obvious place for there to be the first sort of big immunization program involving dyslexia. One dyslexia had been a really serious problem for a long time and affects many people every year. Most people every year their outbreaks every three to four years which are extremely costly for the country. [ UNKNOWN ] Have also been involved in the [ UNKNOWN ] development, there have been in clinical trials there. They were compared to other countries, fairy familiar with this vaccine. So in April 2016 before any other country that they launched big mask organisation program targeting 1000000, 9 and 10 year olds through a school [ UNKNOWN ] You bet you actually involved into a mile dirt bike that will not be required hospitalization. So a big turning point in the story came in November 2017 when Santa Fe made an announcement about The vaccine. They announced that for people who hadn't been affected with dengue before, those people actually had a higher risk of getting severely ill if they had the vaccine. It was no longer recommended that the vaccine be given to people who had not had dengue before. So what Santa Fe didn't communicate was that the risk was very small. There wasn't any sort of Sense of what severe Dengvaxia meant. This caused people to panic messages were going around on social media. People were weighing in without a lot of facts. politicians were calling for investigations, but very quickly exploded into this. You know, storm of. Fear and genuine concern that parents had about their children. There were these reports of children who had been vaccinated who then died. The public attorney's office in the Philippines which is part of the Department of Justice became a sort of the advocates for these people and they started investigating The cases of these children death. And the way they did that was they did autopsy which were shown on TV which they broadcasted Live on Facebook in some cases and the forensic team that was performing these autopsies were saying the organs were swollen breeding and these cases. Two things back there in the process of investigating and filing cases against those that they, you know, allege were involved with the dang vaccine program. So those include Santa fee employees, a lot of government health officers, Some scientists. I categorically deny any wrongdoing. I'm not involved in any corruption. And I'm willing to be investigated. Meanwhile, medical and scientific experts very strongly dispute the claims. They say there's no evidence for this cases being linked to the vaccine, There were people that were pointing out that their forensic examiners were not real pathologists, that the way they were conducting autopsies was incredibly inappropriate. At the moment, the Department of Health in the Philipines is not [ UNKNOWN ]. [ MUSIC ] One really awful side effect of this vaccine scare was that people in the Philipines lost trust in their government, and with that lost trust in vaccines generally. So the coverage rates were already dangerously well in the Philipines, and this event would have pushed it over the edge. In 2018 was a measles outbreak. In 2019, there were more than 40, 000 measles cases in the Philipines. Recently there was a polio outbreak in the Philipines. So, they have seen lots of infectious disease that certain these vaccines care Experts are quick to point out Dengvaxia can have tremendous public health benefits. Especially in countries like the Philippines, like Thailand that have a lot dengue. You know this vaccine could do a lot of good and prevent a lot of dengue cases. The problem is there are. A small population that will not benefit from this vaccine and not only will they not benefit, they could get more sick. So the obvious answer which is to give this vaccine to the people that will benefit from it is not something you can do right now. There is not a quick, inexpensive test the government could use to determine who has done here before and who hasn't. So this is the real strategy here is for ever vaccine against dengue that is really hard for countries to used. For now millions of people who could benefits from the vaccine left away. [ MUSIC ]
